Officially, the name of this popular piano piece (written 1801) is just Piano Sonata No. 14 in C♯ minor “Sonata quasi una fantasia”. It was Ludwig Rellstab who, five years after Beethoven’s death, was reminded of the first movement when he crossed Lake Lucerne at night and saw the moon reflected on the waves. He gave it the name it is now generally known by: Moonlight Sonata.
